1 Option 1: Crystal Lake & Huntley Only: Pre-Registration Online Crystal Lake & Huntley patients can preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Depending on your computer and it’s version, upon your Adobe Reader version, and also depending on your browser (Internet Explorer, Firefox, Chrome, etc) and how browser preferences are set, PDF forms open in different ways.
Normally when you select a form, it opens in the browser window and can be printed from there. However, in some cases, the form has to be downloaded to the computer in order to print. Select the download option and navigate to where you computer saves downloads. Often this is the “Downloads” folder. If you have the option of selecting where to save the form, you may want to simply save it to your desktop. Once the form has been downloaded to your computer, you should be able to print.
